Yep. Kinda a let down isn't it?

I wish we could get a dignified modern version of a transfer cab. But we get this.

It might be a "handle" as some railroad men used to call them.  They are idler cars used to shove cars onto ferries or onto track where weight restrictions did not permit locomotives.  You also see them on tracks where cars are spotted inside buildings.  You do not want a locomotive in there because the exhaust will linger. 

Often, these things were old flatcars onto which the shops added railings.

The one in Original Poster's photograph does not look that old.  This is the first one that I have seen with railings and a brake wheel further in on the car body.

I'd say that was for the brakeman/switchman. so they don't have to stand on the end of the car.  In the middle, with railings on both sides, would be a lot safer with sudden moves or slack action.  It looks like it has all of the valves and piping for air brake control as well, just like a caboose used to.
